Explore strategic approaches to risk adjustment and HCCs

Incorporating risk adjustment and hierarchical condition categories (HCCs) within value-based care can help health care organizations to better manage patient populations. Accurate risk adjustment helps capture a patient’s chronic conditions and the complexity of a patient population.
