How to ensure functionality, aesthetics and durability of posterior composite restorations?

Making posterior composite restorations can often be demanding, if a lot of tooth structure has been lost, in preparations that are located near or below the gingival margin. Often in these cases, when the margin of the preparation is already in the dentin, we think about which material is best to use. In cases lack of a marginal ridge, it is necessary to create a technically precise area of the contact point, which requires a good choice of the matrix system, its placement and isolation of the working field. The durability of the composite restoration also depends on the stability of the adhesive bond, i.e. the prevention of microleakage between the edges of the filling and the tooth. Furthermore, a poorly placed adhesive system (drying of dentin, incomplete polymerization, moisture) can cause the formation of nanoleakage as the most common reason for postoperative hypersensitivity in patients, but also faster breakdown of the adhesive bond. The lecture will explain the sensitive stages of making a composite filling in the back region that can be the reason for failure (loss of adhesive bond, marginal discoloration, poor contact point...). The specifics of the latest universal adhesion systems will be described through a clinical protocol. Their advantages, limitations, clinical work, application possibilities in restorative dental medicine and post-endodontic tooth care will be explained. A number of clinical cases will be presented and how to avoid the most common mistakes in clinical work.

Dr Ivona Bago graduated from the Faculty of Dentistry in 2005. During her studies, she won two University Scholarships, and in 2004 she received the Rector's Award for scientific research in the field of periodontology.
In 2007, she began a specialization in family dentistry at the University Hospital Center Zagreb, and became a family dentist specialist in 2010.
Since March 2010, she has been employed at the Department of Endodontics and Restorative Dentistry of the Faculty of Dentistry in Zagreb as a research fellow in the scientific project "Experimental and Clinical Endodontics".
In 2013, she received her PhD from the Faculty of Dentistry, University of Zagreb. She completed her postgraduate studies in EMDOLA (European Master Degree in Oral Laser Application) at the Sophia Antipolis University in Nice, France in 2014. In 2015, she became a specialist in endodontics and restorative dentistry. In 2016, she became an assistant professor at the Faculty of Dentistry, University of Zagreb, and an associate professor in 2021.
She has been the Secretary of the Croatian Endodontic Society (HED) and the Society's representative to the European Endodontic Society since 2014. She is the head of the course at the postgraduate doctoral study at the Faculty of Dentistry, University of Zagreb and the working course in practical endodontics at the Faculty of Dentistry.
Prof. Bago is regular lecturer at the annual congresses of the HED. She is the author of several scientific and professional papers in domestic and international journals.
